3K a year Works closely with the mental health therapist to develop and implement the service for individuals and families participating in the IIHS program.
Referred clients are experiencing a crisis or transition situation that destabilizes the family and impairs the client's ability to function successfully.
The IHS provides support, referrals (resource brokering), advocacy, and case management services to families in their homes, at schools, or other places in the community.
In addition, this position has responsibility for coordinating with multiple service providers to prevent out of home placement care.
Responsibilities:
Participate with mental health therapist in conducting a comprehensive face-to-face assessment and developing an individualized service plan for client/family.
Provide immediate response to client crises when assigned, and contact appropriate therapist regarding nature and content of after-hours interventions.
Maintain a caseload of nine to twelve clients and meet with clients at their home, children's school, or other locations.
Assist families in crisis situations related to immediate need for food, clothing or shelter, by providing proper referral and case management by facilitating their access to other service providers.
Utilize a family-centered, strengths-based approach to work directly with clients on issues such as (but not limited to) developing prosocial skills, conflict and crisis management, communication, family structure and boundaries, health and safety for children in the home environment.
Attends and presents cases during supervision meetings, and assist in administering outcome measurements to clients/families.
Coordinates service delivery with other service providers and advocates for clients as needed.
Attends community meetings and/or agency/collaboration meetings/trainings as assigned.
Maintains files and other required records in audit condition as required by agency or funding source.
Supports existing agency policies, principles and mission.
Requires:
Bachelor's degree in social work or related field from an accredited university strongly preferred.
Knowledge of social services and community resources required.
Ability to relate to people of various ethnic and socioeconomic backgrounds and work in a multicultural environment Strongly prefer one of the following language capabilities in:
Spanish, French or Amharic.
Ability to relate to people of various ethnic and socioeconomic backgrounds and work in a multicultural environment.
Good verbal/written communication skills, good judgment in performance of duties and responsibilities.
Valid local driver's license with 3 points or less.
Must be detail oriented and able to successfully prioritize and manage multiple tasks while working effectively as a team player.
Requires a high level of professionalism.
Must be able to work independently and as part of a team.

Estimated Salary: $20 to $28 per hour based on qualifications.
