The Care Coordinator is responsible for receiving, prioritizing, and providing triage to incoming assistance requests from clients (veterans, service members, and their family members/caretakers, and referring organizations).
The Care Coordinator is also responsible for maintaining existing partnerships and creating new partnerships as well conducting outreach, in person, and virtually.

Responsibilities:
Answer calls and manage assistance requests for resources, services, and information; provide thorough follow-up and outreach to ensure successful connection to services.
Conduct a comprehensive psychosocial interview with clients for eligibility and completes a baseline mental health screening, works with clients and family members (if applicable) to evaluate client's social, behavioral, and physical health needs.
Enroll clients into appropriate programs and conducts needs assessment, including collecting supporting documentation before providing the appropriate information about referrals to local, regional and national resources.
Assess clients in crisis and resolve conflicts.
Work with clients, caregivers, and/or team to coordinate and develop an effective service plan to help them achieve their goals.
Accurately document all case management activities in data collection tool and reporting software daily.
Monitor and review client cases to include managing multiple service requests and resource referral follow-ups simultaneously.
Research and develop extensive knowledge of military and veteran resources and services in the region, in addition to other local, regional, and national resources.
Provide outreach and engagement, to include presentations and training, in person and/or virtually, to include the ongoing identification of available public and private resources for military, veterans, and their families/caregivers.
Other duties as assigned.
Education:
Bachelor's degree in Human Services (social work, counseling, psychology, or related field) or equivalent experience in human services field.
Experience:
Experience working with at-risk populations is required.
Familiarity with the military and veteran population and related services is preferred.
1-3 years' experience providing case management is required.
Work requires general and specific knowledge of suicide prevention, veterans' services, and case management.
Experience providing outreach and engagement is required.
Transitioning service members, veterans, and spouses encouraged to apply.
Skills:
Excellent oral and written communication skills.
Must be able to manage multiple tasks while working effectively as a team player.
Must have strong communication, coordination, and organizational skills.
Skilled worker in motivational interviewing and ability to meet clients where they are and move them quickly to community resources.
Provide excellent customer service to those seeking assistance.
Excellent computer skills with proficiency in Microsoft Office Suite, Zoom, Teams, and other technology platforms.
Proficient with data collection and reporting procedures.
Abilities:
Ability to relate effectively to diverse individuals.
Effectively interact and work collaboratively in a flexible work environment with a variety of constituents including CEO, board members, clients, family members, caretakers, interns, volunteers, paid staff, and community partners.
Ability to express ideas with clarity, confidence, and sensitivity.
Must be able to maintain confidentiality involving clients in accordance with organizational and federal guidelines.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
