This position is responsible for the on-going and long-term client supportive services of now housed, previously homeless clients, to ensure individuals stay stably housed.
As well as, work to move referred clients through the HIP application process, and/or housing search and leasing process until successfully housed, as needed.

Responsibilities:
Advocate on behalf of clients for needs, services and supports while working with those in the program and in the community through a Harm Reduction perspective and Housing First model.
Adhere to all HIPPA and client confidentiality requirements for the contract, EveryMind and all electronic health records while treating clients ethically, with a client-centered trauma informed supportive experience.
Meet face to face with clients in community settings and/or clients home following clients assigned acuity tier for the number of services ensuring all visits meet standards for reimbursement.
Service Coordinators are expected to meet assigned acuity and respective billing monthly.
Any exceptions are communicated to Director as soon as possible and all staff efforts to ensure the highest level of billing reimbursable is met must be documented.
Effectively utilize the county electronic health record database, HMIS, to enter client contacts and information following all HIP and county guidelines related to timing, entry format, reports, and updates.
HMIS is a critical aspect to tracking, billing and communication about the clients.
Inability to adhere to timelines, data quality and accuracy requirements would be grounds for termination.
Maintain client paper chart/case records securely and up to date (annually renewals included) in adherence to HIP guidelines and EveryMind standards.
Paper charts should always be ready for a site audit or transition of Service Coordinator.
Complete all additional tasks related to monthly HIP tasks in coordination with visit tracking to ensure accurate and on time billing and HIP monthly and then bi-annual reports and other program requirements as assigned.
Complete all HIP program client specific paperwork per contract to assist client in maintaining their HIP voucher or as required for reporting purposes.
Direct transportation of clients in accordance and in compliance with agency policy based on client need and ability.
This can include arranging alternative transportation options, when approved by Director and dependent on situation.
Submits reports and complies with Director requests for data reporting as needed.
Works with and encourages good communication among the agency and staff at collaborative partners.
Completes and engages in trainings as needed to support client needs and effectively perform duties.

Education:
Bachelor's degree or equivalent experience required.
Experience:
At least two year of experience in providing services to low-income, homeless, psychiatric, substance involved or co-occurring adults.
Skills:
Excellent oral and written communication skills.
Must be able to organize and prioritize.
Experience with Microsoft Office platforms, particularly Excel.
Accurate data entry skills.
Independent and self-directed.
Abilities:
Effectively interact and work collaboratively in a flexible work environment with a variety of constituents including CEO, board members, clients and family members or caretakers, interns, volunteers, paid staff, and community partners.
Ability to express ideas with clarity, confidence and sensitivity.
Must be able to maintain confidentiality involving clients in accordance with organizational and federal guidelines.
Familiarity with working and hiring interns and volunteers.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
